Please arrive at least 15 minutes before your appointment time to fill out your registration paperwork.

You can complete your Patient Registration Form in advance by clicking the button below.

Please bring these items to your appointment:

  • Referral from your General Practitioner - without a valid referral you are unable to claim the Medicare rebate, however, we are still happy to see you without one

  • Medicare card

  • Private health insurance details

  • Health care or DVA card

  • TAC or Workcover details - including claim number and approval letter

  • Imaging – X-rays/CT scan/ MRI scan/Ultrasound

  • List of your up to date medications and medical conditions
